Transition Counselling
Transition Counselling is helpful when a loved one is leaving treatment. It helps everyone clearly express their expectations, needs and wants and eliminates confusion. Safety plans and relapse prevention plans are communicated clearly and put into place. There is a clear course of action in the event of a slip or relapse.
The process consists of the counsellor facilitating individual sessions for each of the family members and hearing the wants and needs of everyone. The counsellor then creates a plan and puts everything on paper so each person can see their role. After this is complete, the family gets together, either in person or online, and discusses and fine-tunes the plan. The counsellor then puts the plan on paper and sends it out to everyone.
